Use this form to register. Please remember that whenever you choose to post your E-mail address on the internet, you may be contacted by unwanted solicitors (It has happened!). This may include persons offering paid search assistance. Buyer Beware! Be leery of anyone who contacts you and claims to be searching on behalf of the party you are looking for. Be especially leery of anyone who is working on behalf of the big search companies that are touted on the TV talkshows. You may want to consider using a blind e-mail box for registry use. You can easily sign up for one free at Websites such as: Hotmail, Yahoo!, Mail City, Mail.com, Netscape or GMail.

No mailing addresses or phone numbers will be posted on the registry; they will only be kept offline as contact information. Remember to keep us posted of any changes in your E-mail address, phone number, and/or mailing address! Also, please review your registration within a week to make sure it was posted correctly.

THIS REGISTRY IS FOR ADOPTIONS THAT TOOK PLACE IN THE STATE OF NEVADA ONLY!! However, if there is any tie to Nevada (evidence that birth or adoptive families may have lived in/currently living in Nevada, etc.) the information will be posted. Please make it very clear in the comments area what the tie to Nevada is specifically.

A Quick Note about Identifying Information in this Registry.  Please feel free to submit any identifying information that you have about your birthrelatives to the registry. HOWEVER, we will not post the full names of birthparents or the amended names of adoptees, as this registry can be viewed publically by anyone who accesses the web pages. We will post partial names, however. For example, Joan Smith would be Joan S****. Identifying information will be kept private with your registry submission form. WE WILL post original birthnames of adoptees (provided by either the adoptee or the birthfamily), and we will post the names of birthrelatives IF they are provided by the person themselves (i.e. a birthmother may have her maiden name posted, etc.). This is simply for privacy.

We encourage you to submit your phone number and address with this registration, however this is not required. Again, these will not be posted on the website for public viewing.

PLEASE NOTE THAT YOUR ADDRESS AND PHONE NUMBER WILL BE KEPT CONFIDENTIAL, UNLESS YOU AGREE TO ITS RELEASE TO A PERSON MAKING AN INQUIRY ON YOUR REGISTRATION.

PLEASE FILL IN ALL INFORMATION YOU HAVE; IT IS NOT NECESSARY TO COMPLETE EVERY ITEM.

NOTE: If you have already registered and do not see your registration posted on the web site, please DO NOT re-register using this form. Please check the last date for which registrations were posted on the registry homepage. If you have checked and still feel that your registration should already be posted, re-register using the form below and state that you are re-submitting in the comments box. For other inquiries, contact us by E-mail.

You may remove yourself from this registry at any time for any reason (or no reason). Simply notify Nevada Triad of your desire.
